What is Aptamil AR Anti Reflux formula?

Aptamil AR is a thickened European anti-reflux baby formula for infants from birth who bring up milk frequently. It is thickened with locust bean gum, a plant fibre also called carob bean gum, which makes the feed heavier so it is less likely to come back up. This listing is the large 800 g can — about 210 fl oz of prepared formula, roughly 42 bottles of 5 fl oz.

The regulatory category matters here. Aptamil AR is sold as a Food for Special Medical Purposes — the EU classification for a food designed to manage a specific condition, intended to be used with healthcare-professional advice rather than picked off a shelf. It is not a premium everyday milk and it is not a general "sensitive tummy" formula. It is also not suitable for premature infants.

This particular 800 g can is the German-market pack, produced by Nutricia Milupa GmbH. Our 400 g can comes from a different national market — it is the Lithuanian-market pack, made in the Netherlands — but the two cans are essentially the same formula in two pack sizes. Both are thickened with locust bean gum, both are built on the same lactose, skimmed milk and whey base, both carry the same GOS and FOS prebiotics and the same fish oil, and both declare the same allergens: milk, soy and fish. Only three label details genuinely differ: this 800 g carries a third whey fraction, while the 400 g lists high oleic sunflower oil in its vegetable oil blend and a second form of vitamin C. Everything else that reads differently between the two labels is simply the two countries' naming conventions for the same vitamins and minerals.

Where it sits in the feeding journey: AR can be used from birth as a complete formula, and parents typically arrive at it after their doctor or health visitor has confirmed the spit-up is frequent reflux rather than ordinary posseting. Because the feed is thickened, it needs a fast-flow, single-hole teat and a preparation routine that differs from ordinary formula in ways that matter — read the preparation section before your first bottle. The ingredient deck is built on a dairy blend of lactose, vegetable oils (palm, coconut, rapeseed and sunflower), skimmed milk and three whey fractions — demineralised whey, whey concentrate and whey protein — followed by galacto-oligosaccharides, the carob bean gum thickener, fructo-oligosaccharides, oil from Mortierella alpina for ARA, and the full vitamin and mineral spread. Declared allergens are milk, fish and soy — the soy comes from the lecithin emulsifier.

One correction worth making plainly: this is not a reduced-lactose formula. The published panel shows 7.0 g of lactose in 7.3 g of total carbohydrate, so lactose is essentially the whole carbohydrate load. Anti-reflux works here through thickening, not through removing lactose. If lactose is the problem your doctor has identified, AR is not the answer.

The science

How does a thickened anti-reflux formula work?

Aptamil AR contains locust bean gum, a fibre milled from carob seeds. Once it has been mixed and allowed to stand, the gum swells and raises the viscosity of the feed — a heavier, thicker liquid is simply harder to bring back up than thin milk.

That is the whole mechanism, and it explains two things parents often find surprising. The first is the five-minute standing time after mixing: the gum needs those minutes to swell. Feeding immediately gives your baby thin formula and largely defeats the point of buying it. The second is the teat — a thickened feed will not flow through a standard slow-flow hole, so Aptamil specifies a fast-flow, single-hole teat.

It also explains the manufacturer's firmest preparation rule. Because the feed thickens over time and can clog or set unevenly, Aptamil's own safe-preparation guidance says not to use bottle warmers, formula preparation machines or similar dispensing devices with AR. Preparation

How do you prepare Aptamil AR Anti Reflux formula?

Aptamil AR is prepared differently from ordinary formula and the differences are not optional. Cool boiled water to about 40°C, pour two-thirds of the measured water into a sterilised bottle, add the exact number of level scoops, shake vigorously for 10 seconds, add the remaining water and shake again, then let the bottle stand for 5 minutes so the gum thickens, shake briefly, check the temperature and feed through a fast-flow single-hole teat.

Sterilise everything

Thoroughly clean and sterilise the bottle, teat and ring, and wash your hands before you start.

Boil and cool to 40°C

Boil fresh drinking water and let it cool to about 40°C, the temperature Aptamil specifies for this thickened formula.

Pour about two-thirds of the measured water into the sterilised bottle before any powder goes in.

Level each scoop against the scraper and add the exact number from the feed table — never press, heap or add an extra.

Leave the bottle to stand for 5 minutes so the gum thickens, shake briefly, check it is about 37°C and feed through a fast-flow teat.

Do not use a bottle warmer, a formula preparation machine or any similar dispensing device with Aptamil AR. This is the manufacturer's own instruction, and the thickener is the reason for it — a feed that thickens on a timer can set unevenly or clog. Prepare every AR bottle by hand, fresh for each feed, and give it to your baby straight away. Never microwave it, never add extra scoops or a second thickener, never combine it with antacids, and never reuse leftovers — discard anything unfinished after 2 hours.

A note on water temperature. Aptamil specifies water at about 40°C for this formula, and that is the instruction printed on your can. It is set deliberately — hotter water damages the heat-sensitive vitamins this formula is built around.

Many health authorities, including the NHS and WHO, advise 70°C or hotter for any powdered formula, because powder is not sterile and that temperature kills bacteria that can occasionally be present. The difference matters most for newborns, premature babies, and babies with weakened immunity. If your baby is in one of those groups, ask your paediatrician or health visitor which to follow for your situation.

You will need a different teat

Thickened formula will not flow properly through a standard newborn slow-flow teat. Aptamil specifies a single-hole, fast-flow teat for AR. If your baby seems to be working hard at the bottle and getting frustrated, the teat is the first thing to check — before you conclude the formula is not working.
